What are your locations and how many counties do you serve?

The agency offers 11 service sites in 8 southeastern counties (Bell, Clay, Jackson, Harlan, Knox, Laurel, Rockcastle and Whitley). Services available 8:00am – 4:30pm M-F, hours of operation and staffing patterns are based on needs assessment information

Is there a specific referral form?

No. Some programs/services have a referral form that they like to use, but anyone can make a referral by simply calling the office closest to them.

Do I need an appointment to receive services?

Appointments are not necessary when seeking services. All of our outpatient offices offer open access which means that during certain windows of time throughout the day, individuals can walk in for therapy services and be seen. Please note that open access does not apply to all services at this time i.e. psychiatric. For more information please contact the office nearest you.

Is there admission criteria to receive services?

Each program has identified admission criteria to determine access to a service. If you are interested in learning more about a particular service/program, please contact the office nearest you.

What information is needed for the first appointment?

Insurance Card(s), Social Security Card, Driver License, List of Current Medications, Custody Papers (if applicable).

What if an individual does not meet criteria for a program, but still needs services?

If an individual does not meet criteria for a specific program, he/she can be referred to other programs and/or services that may better meet his/her needs. Referrals to appropriate community resources can also be made.

Who can make a referral?

Anyone can make a referral for services. A person can self-refer or be referred by another party i.e. family member, physician, friend, etc.

What information is needed to refer a patient?

Demographics such as name, date of birth, contact information, insurance, current medications, custody papers (if applicable), last doctors note if relevant to referral.

Do you offer virtual service options?

Yes. Virtual/telehealth service options are available. For more information please contact the office nearest you.

What insurance is accepted?

We accept all kinds of insurances. (Commercial, Medicaid, Medicare, Military, Self-Pay). No one is turned away for an inability to pay.  We Offer a Sliding Fee Based on Income and Family Size (Sliding Scale)

What modalities and/or practices are utilized during treatment?

Each service provided is customized to the individual receiving the service. Treatment modalities often used include, but are not limited to: Cognitive Behavioral Therapy, Trauma Informed Care, ACRA, Seven Challenges, Dialectical Behavioral Therapy, Motivational Interviewing, Eye Movement Desensitization Reprocessing (EMDR), etc. All of these modalities are evidence based and have been clinically proven to be effective. All CRBH staff are trauma aware. Direct service staff are trained in Trauma Informed Care. For more information about specific modalities, please contact the office nearest you.

Who do I call if I am experiencing a crisis?
988 is the three-digit telephone number in the United States, designated as the Suicide & Crisis Lifeline. The number provides immediate access to crisis counseling and mental health resources. By dialing 988, people in the United States can be connected to a trained crisis counselor who can provide emotional support, resources, and referrals to local mental health services. The 988 number was created to provide a simple and easy-to-remember alternative to the National Suicide Prevention Lifeline’s previous 10-digital number (1-800-273-TALK) that is still in use. The goal of the 988 number is to improve access to mental health resources and reduce the stigma surrounding mental health issues and suicide.
